ADMISSION REQUIREMENTS FOR REGULAR SCHOOLING PROGRAM

ADMISSION REQUIREMENTS for Continuing (old Student)

  1. Duly Accomplished ApplicationForm
  2. 2 X 2 ID Picture (2 pieces with white background) or jpegformat
  3. Proof of payment (if bank deposit, submit the deposit slip & bankdetails)

ADMISSION REQUIREMENTS for Kindergarten

  1. Duly Accomplished ApplicationForm
  2. 2 X 2 ID Picture (2 pieces with white background) or jpegformat
  3. Clear Copy of the Birth Certificate (Minimum Age requirement: 5 Years Old by August of that school year)

ADMISSION REQUIREMENTS for Transferees Grades 1 to 10

  1. Duly Accomplished ApplicationForm
  2. 2 X 2 ID Picture (2 pieces with white background) or jpegformat
  3. Submit the original Report Card w/ 12 digits Learner’s Reference Number(LRN)
  4. Certificateof Good Moral Character (local) or Certificate of Eligibility or Certificate of Completion (from school abroad)
  5. Clear Copy of BirthCertificate
  6. Diagnostic Test Fee of P200 and Interview

 

ADMISSION REQUIREMENTS FOR HOMESCHOOLING PROGRAM 

ADMISSION REQUIREMENTS for Continuing (old Student)

  4. Parent’sProfile

 

ADMISSION REQUIREMENTS for Transferees Grades 1 to 10

  2. 2 X 2 ID Picture (2 pieces with white background) or jpegformat
  3. Submit the original Report Card w/ 12 digits Learner’s Reference Number(LRN)
  4. Certificateof Good Moral Character (local) or Certificate of Eligibility or Certificate of Completion (from school abroad)
  5. Clear Copy of BirthCertificate
  6. Form 137 (from school abroad)
  7. Parents/Tutor’s Profile (clear copy of any:Credentials/Accreditation/Diploma/TOR)

 

Other requirements:

  1. For Student’s Abroad: All requirements # 1 to # 7 including Form 137 is a must and TransferCertificate
  2. For Independent Homeschoolers: All requirements # 1 to # 7, Acceleration/ P.E.P.T. / Validating Test Result (in the absence of the Report Card and Form137)
  3. For SpeD student: All requirements # 1 to # 7, Evaluation or Assessment of the DevelopmentalPedia
  4. Diagnostic Test Fee of PhP 200.00 for new student.
